Information about Calcium-L Threonate

Question: I want to know more information about Calcium-l-threonate.

Answer: Calcium L-threonate is the product of calcium in the form that has been developed for use in osteoporosis.
In addition to Calcium-l-threonate provides Calcium (a substance helps build bones) and also provides l-threonate which helps the synthesis of collagen. 

Calcium-L-Threonate has a role in bone formation process of osteoblast. One tablet of calcium-l-threonate products weighs 972 mg.
Which is amount of calcium-l-threonate 775 mg and it includes calcium100 mg and L-threonate 675 mg.
The recommended dose is 1-2 tablets two times a day (2-4 tablets per day).

Can cyproheptadine be given 1 tablet per day for appetite?

Question: Dosage of cyproheptadine for appetite usually 1 tablet 3 times a day. Is the effectiveness of appetite reduced or not, if it is given orally 1 tablet once daily?

Answer: cyproheptadine is an anti-histamine. It is used to treat allergy symptoms. Cyproheptadine has been used to stimulate appetite.

There are reports of clinical studies in healthy volunteers about efficiency in stimulating appetite. The dose of cyproheptadine used is 4 mg 3 times a day. It is found that the volunteers had appetite and weight gain when compared to placebo.

However, no information on the effectiveness of cyproheptadine for stimulate the appetite, when the drug is eaten 4 mg once daily.

Will high dose of foliamin can affect the baby?

Question: I am pregnant and have eaten foliamin 1 tablet two times a day, morning and evening for about one month. Later I knew that the doctor ordered me to take one tablet once a day after breakfast, only. Now I worry about what happens to the pregnancy. Will it affect the unborn child or not?

Answer: Foliamin or Folic acid is used in pregnant women for prevention of disorders of the brain and spinal cord of baby. The recommended dose in pregnant women is 0.4-0.5 mg per day. The questioner did not specify the dose received, but it is expected to be 5 mg per tablet which is commonly used, especially in pregnant women with risk of neural tube defect of baby.

A follow-up study of the intellectual development of infants born from mothers who received folic acid in doses of 5 mg per day, higher dose of 5 mg per day, compared with mothers who did not receive folic acid during 14-18 weeks of pregnancy.

The data found that children born from mothers who received folic acid in doses of 5 mg per day was associated with improved understanding of the language and improved language usage when compared with children born from mothers who did not receive folic acid, and children born from mothers who take folic acid in high doses over 5 mg per day did not develop increasing more than children born from mothers who received foliamin 5 mg per day

However, the study did not report the danger caused by folic acid in high doses over 5 mg. Information on toxicity of folic acid when given in high doses is unknown. But it is reported that healthy volunteers who received high doses of about 15 milligrams of folic acid per day is associated with behavioral changes such as restlessness. And they increased response to stimuli and had sleep disorders.

Zinc chelate and biotin for hairloss

Question: I have hair loss and I had bought zinc chelate 15 mg and biotin 150 mcg. But I do not know how to eat, and how long can they be eaten? Can they solve the problem of hair loss?

Answer: Zn (zinc) is a mineral used in body processes. It is used as catalysts in combination with various enzymes which is vital to the functioning of organs such as growth, creating various tissues, including the proliferation of hair follicle cells and so on.
Generally the body gets zinc from diet as normal. But a lack of zinc can be found in people who have the problem of absorption.
The zinc deficiency is one cause of the hair loss and thinning hair spread over the entire head. Zinc chelate is zinc in the form of binding with organic molecules such as amino acids to increase absorption.
There is currently no reliable study yielded a clear result of the use of zinc to treat hair loss, including the dose and duration. But there is a study about the levels of zinc in the blood in patients with diffuse thinning hair over the entire head which showed that the amount of zinc did not differ from the control group (=normal people). But in a study of patients with patchy hair loss which have low blood zinc levels. To receive zinc supplements for replacement which can help improve symptoms of their hair loss.

Biotin (biotin) is classified in the same group with thiamine (vitamin B). It is often sufficient in food. So biotin deficiency is rarely happens. The body needs to take biotin in the process of producing new cells, especially cells that are constantly dividing, such as hair and nails. Therefore biotin deficiency can cause hair loss and inflammation of the skin.

There is currently no convincing evidence about dose, duration and effectiveness obtained from the use of biotin treatment of hair loss as well.

The more information about hair loss: It may be caused by many factors such as a lack of nutrients, genetics, stress, chronic hair pulling, hair decoration, some skin diseases and the use of some drugs for a long time.
This may require further research to find the causes of hair loss.

Can people with high cholesterol eat coconut?

Question: Do coconut water and coconut meat affect blood cholesterol especially in patients or people with high blood cholesterol?

Answer: Generally, fat in food can be divided into several types: saturated fat, trans-fat and cholesterol. Blood lipid levels are different depending on the type of fat that have been eaten.

Coconut water and coconut meat contain the following components:
- 100 grams of coconut water provides 16 kilo calories of energy. The main component is water and there are other components such as carbohydrates 3.9 g., protein 0.1 g., vitamins and minerals.
- 100 grams of Coconut meat provides 81 kilo calories of energy. The main component is water and there are other components such as 5.9 grams of fat, 3.8 g of carbohydrates, 3.2 g of fiber, 1.8 grams of protein, vitamins and minerals.

The data shows that carbohydrates are the main ingredient in coconut water and there is a little fat. So eating coconut water can affect blood sugar levels more than blood lipid levels.

But the coconut meat will have major component of fat and carbohydrates. The fat from coconut is mainly saturated fat, which can increase blood cholesterol levels. It is recommended for normal people to eat saturated fat no more than 7% of calories received per day, or about 16 grams. In addition, coconut meat also contains sugar, which can affect blood sugar levels as well.

If possible, it is the best for patients with high cholesterol that they should avoid eating coconut meat.

Question: Diarrhea and vomiting affect the action of oral contraceptives. Is it true or not?

Answer: The data found that diarrhea and vomiting affect the action of oral contraceptives. The diarrhea that results in reduced effectiveness of oral contraceptives is severe diarrhea. It does not have to define definition of severe diarrhea (such as yet undefined number of time that defecation), and there is no evidence or clinical studies about the efficacy of the drug to decline that would affect pregnancy or hormone levels when severe diarrhea.

There is also a suggestion that if the diarrhea for at least 48 hours during taking oral contraceptives. It is not necessary to eat more contraceptives and do not need to use another more contraception.

But if you have diarrhea for up to 48 hours or longer, it is recommended to eat the next tablet of contraceptives as usual, but using drug together with other methods of contraception, such as using condoms, or avoiding having sex as long as until having eaten a pill every day for at least seven days after stopped diarrhea. And it may require more emergency contraceptives in some cases that have sex without protection.

Can I take zinc and vitaminC for a long time?

Question: How useful are zinc and vitamin C 1,000 mg to the body? And can I take them for a long time?

Answer: In general, food helps to absorb vitamins and minerals. So vitamins and minerals should be taken with or after food. However, the body will usually get vitamins and minerals from food. Therefore, vitamin or mineral supplements should be eaten only when necessary as in the case of the elderly. Elderly people will have reduced absorption of food. It may cause a deficiency of vitamins or minerals. But if there is no risk of lack of vitamins or minerals, vitamins and supplements should not be eaten. Because it may get the vitamins or minerals in excessive amounts until it can adversely affect the body.

Zinc is a mineral that used in the process of the body. It is used as catalysts in combination with various enzymes which is important to the functioning of organs such as growth, tissue regeneration and etc. Generally the body gets zinc from the diet, but lack of zinc can be found in people with problem of absorption.
Each person needs a different amount of zinc based on gender and age. Zinc usually should not be taken more than 40 mg per day.
To get zinc in excessive quantities, it may interfere with the functioning of various systems such as metabolism of copper and iron, or it may cause nausea, vomiting, abdominal pain, diarrhea and etc. However, eating zinc for a long time should be in the care of a physician.

Vitamin C normally should be taken in 50-60 milligrams daily for adults, but in certain conditions such as pregnant women, smoker, people with frequent colds, it should take vitamin c more than normal.
There were no reports about the toxicity of vitamin C. The most common side effects occur in gastrointestinal tract (when vitamin C is eaten in high doses), such as abdominal discomfort, diarrhea.

Since most of the nutrients and vitamins can get from the diet. If we eat nutritious food which contains 5 groups of carbohydrate, protein, fat, vegetables and fruits. Vitamin or supplements may not be needed, because it may have the opportunity of dangerous from excessive eating.

Is it dangerous to take daflon 2 tablets once daily?

Question: I am a little varicose vein, now I'm taking a drug named daflon two tablets once daily after breakfast. Is it dangerous?

Answer: Daflon 500 mg® contain diosmin 450 mg and hesperidin 50 mg for the treatment of chronic venous diseases such as hemorrhoids, varicose vein, etc.
The recommended dose of daflon for varicose vein is 1000 mg per day (1 tablet orally two times a day, or two tablets once daily).

There were quite a few reports of adverse reactions which were found include gastrointestinal such as nausea, vomiting, abdominal pain, diarrhea, dyspepsia, and neurological symptoms such as dizziness, insomnia, headache. And there were no reports of serious adverse reactions.

However, medication should be considered appropriate and necessary to use, in order to avoid side effects that may occur.

Is ranitidine used to treat people with allergies?

Question: What is indication of ranitidine 300mg? Is it used to treat people with allergies (urticaria) or not?

Answer: Ranitidine is a gastric proton-pump inhibitor. It is used to treat gastric and duodenal ulcers and GERD (gastroesophageal reflux disease).
Ranitidine is H2-receptor antagonist which inhibits the action of histamine on H2-recepetor. Histamine is important substance associated with allergic reaction, swelling, itching through the mechanism of action on H2-receptor.

Thus ranitidine is brought in combination with other medications to treat some allergic reactions. Such as ranitidine is used in combination with diphenhydramine which is H1-receptor antagonist, to treat urticaria.
This method is more effectiveness than single medication treatment.

Which antihistamines can I eat while I am pregnant?

Question: I am pregnant and now I catch a cold. Which antihistamines can I eat?

Answer: Antihistamines are generally supplied in the market including chlorpheniramine (CPM), loratadine, cetirizine and fexofenadine. The U.S. Food and Drug Administration has identified the risks that may occur from drug using in pregnant women, it is 5 category A, B, C, D and X. Sequentially, category A is the most safety and category X is the most dangerous.

Chlorpheniramine (CPM) and fexofenadine are classified in category C in the whole period of pregnancy, loratadine and cetirizine are classified in category B in the whole period of pregnancy.

Therefore, antihistamines are not a contraindication in pregnancy, but they should be used as needed. Cetirizine and loratadine are the kinds of drug that are safer than other drugs.

Why do I feel a burning sensation in the eyes after the eye drops using?

Question: Why do I feel a burning sensation in the eyes after using the eye drops that contains antazoline and tetrahydrozoline?


Answer: The eye drops contains antazoline and tetrahydrozoline. It can make blood vessel contraction and acts as antihistamine. The eye drops is indicated for the treatment of allergic reaction, irritation or itching and swelling of eyes due to allergies to chemicals, pollen or weather.

The acidity (pH) for stability of the eye drops is about 5.4, and pH of human tears normally is 7.4, so this may cause eye irritation or eye burning.
However, product of eye drops may have the pH in the range 4.5 to 11.5, because the human tears can dilute the acidity of the drug into normal (=7.4). So, after using the eye drops for a moment, symptoms like burning eyes are gone.

There are also reports that antazoline and tetrahydrozoline cause adverse drug reaction such as, eye irritation and burning sensation after using the eye drops. If the symptoms of burning eyes occur unusual, severe and occur for a long time, it is advised to stop using the drug immediately. And you should return to the hospital where you received treatment in order to be determined the exact cause, or a change to another drug.

I don't eat the birth control pill at the same time everyday, what will it affect?

Question: I am taking the birth control pill every day at bedtime, but rarely on time.
Sometimes, I eat early no more than two hours. Sometimes, I eat late no more than two hours. What will it affect?

Answer: Eating birth control pill that is not punctual. It may affect the efficacy of oral contraceptives because drug levels in the blood are not steady. So if you want fully effective contraceptives, you should take your birth control pill at the same time every day.

Is coffee involved with calcium absorption?

Question: Coffee is involved with calcium absorption, is it yes or not?

Answer: Drinking one glass of 180 mg of coffee, it would lose the equilibrium of calcium to 4.3 mg.

The caffeine in coffee will reduce calcium absorption in the intestine. The caffeine is a mild diuretic and increase sodium excretion in the urine. As a result, calcium is drawn out in urine as well.

It is not found that coffee drinking has a negative effect on calcium balance if the calcium intake is not less than 600-800 mg per day.

However, coffee drinkers tend to avoid drinking milk (milk is one cause of reducing the absorption of calcium as well). In summary, when drinking lots of coffee, but not eating enough calcium, it may lose the balance of calcium.

Drug interaction between antibiotics and oral contraceptive

Question: What is the mechanism of drug interaction between antibiotics and oral contraceptives?


Answer: There are several mechanisms of drug interaction between antibiotics and oral contraceptives, depending on the type of antibiotics.

Such as:
Penicillins and Tetracyclines will reduce the absorption of oral contraceptives. By inhibiting bacteria in the intestine that affects an enzyme used in the process of enterohepatic recirculation. And it resulted in decrease in level of oral contraceptives.

Rifampicin is an enzyme inducer. It makes an enzyme in the liver to eliminate estrogenic and progestational component in oral contraceptives from the body faster.

How can the cotton wool be used most effectively when it is moistened with alcohol?

Question: At what time should the cotton wool ball be used most effectively when it is moistened with alcohol?

Answer: Alcohol (ethyl alcohol, isopropyl alcohol) in the strength 70% in water is used to clean the wound.

Alcohol in the strength 70% in water has fast-acting bactericidal, but short-acting. It should be used immediately. Cotton wool moistened with alcohol should not be left for long, because alcohol evaporates quickly and will be diluted and less effective.

Alcohol acts by precipitation process of protein. Isopropyl alcohol is slightly more potent than ethyl alcohol. Some places may prepare the isopropyl alcohol slightly lower strength than ethyl alcohol. If the two kinds of alcohol were used in the equal strength, isopropyl alcohol may still be effective a little longer than ethyl alcohol.

However, it is recommended that cotton wool moistened with alcohol should not be left for too long. It should be used immediately, whether it is isopropyl alcohol or ethyl alcohol (or any other kinds of alcohol). And all of them should be kept in tightly closed containers.

How long can the drug be stored after opening the container?

Question: How long can the drug for external use in the emergency room of the hospital (such as a bottle of 450 ml of povidone-iodine, alcohol, ammonia, gentian-violet) be stored at temperature room after opening the container?

Answer: Expiration date of pharmaceutical is the expected time of properties of pharmaceutical will be remained under storage condition as defined in the Pharmacopoeia. The expiry date determined by the manufacturer could not be applied as the expiry date of pharmaceuticals repacking in containers that are different from the original.

But another way to calculate an expiration date is beyond use date.

Beyond use date is the date that pharmaceuticals should not be used anymore. It is used to determine the age of pharmaceuticals in the following cases.  
        - Pharmaceutical formulations are prepared for use in hospital only.
     - The repacking from the container of manufacturer.
     - There is no information about the stability or expiry date of pharmaceuticals. .

The beyond use date of the pharmaceutical repacking should be given a period of 25% of the rest from the date of repacking until the expiration date of pharmaceutical product in the original container defined by the manufacturer, or generally six months after the date of repacking, by choosing a shorter period of time.

Example: Expiration date on the label that is defined by manufacturer is December 12, 2015. Date opening medication is December 12, 2014. The period from December 12 2014 to December 12, 2015 was a period of 12 months and 25% of 12 months is three months, so the expiration date should actually be three months from the date of opening the container, or it was April 12, 2015.

Pharmaceuticals are generally defined to expire six months after opening the container. But the method which gives the shorter expiry date generally should be selected to determine expiry date more than another method. So the expiry date of this example is three months, not six months.

However, consideration of the expiration of pharmaceuticals must regard the characteristics of pharmaceutical products such as changing color, turbidity or precipitation and etc. Because if external features of pharmaceutical products are changing, although they have not reached beyond-use date specified, the pharmaceutical products should not be used anymore.

In case of opening the container of povidone-iodine, alcohol, ammonia, gentian violet, They are determined the beyond use date, as well as pharmaceutical repacking, as mentioned above. Also keep these pharmaceutical products in the environment with temperature and humidity controlled as defined in the Pharmacopoeia or is listed on the label.

If there is no information about temperature and humidity of some pharmaceutical products, They are generally stored at room temperature that has relative humidity less than 75%, and they should be kept in their original container and cover tightly when not in use.

Answer: K-Y jelly is a topical emollient formulation. The expiry date is  defined by the next six months after opening.


But another way to calculate an expiration date is beyond use date. Beyond use date is the date that pharmaceuticals should not be used anymore. The beyond use date of pharmaceutical packaging or after opening to use, is calculated from 25% of the rest from the date of opening the container until the expiration of pharmaceuticals in the original container labeled by the manufacturer.

Will amoxicillin reduce effectiveness of oral contraceptive?

Question: I have a sore throat, swollen and purulent. The doctor prescribed me amoxicillin. But now I am taking oral contraceptives. 
Do I suspect that amoxicillin will reduce the effectiveness of oral contraceptive or not?

Answer: There is the data about drug interaction between contraceptive and amoxicillin that may reduce effectiveness of oral contraceptive.

In the past, a few clinical cases had been reported that had failure to prevent pregnancy when they ate amoxicillin with oral contraceptive. However, there is no study quality and reliable enough to be supported.

In such cases, if the patient becomes infectious diseases in the upper respiratory tract, which is necessary to take antibiotics. It is recommended that patient should be taking received amoxicillin.

Allopurinol and Colchicine in Gout

Question: When do we take allopurinol and colchicine in gout?

Answer: Colchicine is used for the treatment of acute gout attack by reducing pain and inflammation. The mechanism of action is unknown. It is believed that the drug involved in the reaction of the body to respond to the crystals of uric acid. The drug is used only for short periods and stopped when the pain relief. This drug does not slow the progress of gout.

Allopurinol is the drug that reduces uric acid levels in the blood. It inhibits xanthine oxidase which is enzyme in the process of producing uric acid. The Drug used in patients with high blood levels of uric acid, but no effective treatment for acute gout attack.

Norfloxacin and antacid

Question:
1. What is the reason why do not use norfloxacin with antacids.
2. Is it eaten with milk, yes or not?
3. The doctor prescribed this drug to me several times, each time I never take medicine out. What will happen and what effect is it?

Answer:
1. Antacid is the drug which contains aluminium, magnesium or calcium. They will reduce the absorption of drugs in group of fluoroquinolones (norfloxacin is in this group). So norfloxacin should not be eaten together with antacids.
Norfloxacin should eat at least two hours before or after eating antacids. The other drugs in fluoroquinolones may vary slightly. Mostly it is recommended to eat before antacids at least two hours, but period after taking antacids may be different, such as ciprofloxacin and lomefloxacin should be eaten after antacids for at least six hours.

2. Dairy products include milk and foods that are made from milk. And milk also contains high amounts of calcium. So, they can reduce the absorption of norfloxacin.
Norfloxacin should be taken before meals or milk products for at least one hour and after a meal or dairy products for at least two hours.


3. It is important to take all of your medication as instructed by your doctor, even if you are feeling better in a few days. Not finishing the complete dose of norfloxacin may decrease the drug's effectiveness and increase the chances for bacterial resistance to norfloxacin and similar antibiotic.

ACEI , Microalbuminuria, Serum creatinine

Question: Antihypertensive (ACEI group) can slow the deterioration of the kidney in DM (diabetes mellitus) patients with renal impairment, is it true or not? And the microalbumin test has advantages over serum creatinine test, is it yes or not?

Answer: Currently, there is evidence believed that ACEI drugs can slow the deterioration of the kidneys as well as reducing the complications of the cardiovascular system in diabetes patients with microalbuminuria.

Microalbuminuria is a condition which albumin is excreted in the urine 30-300 mg / day. It is a sign of kidney dysfunction in the early stages that can be detected by common method.


Although, the serum creatinine will be monitored easily, but it has a low sensitivity to see the efficiency of the kidney. The serum creatinine will increase, only when the kidneys are already degenerative extremely. In addition, there are many factors that influence serum creatinine values, such as muscle mass, age of patient, protein content in the diet and so on.

Erythromycin and people with abdominal pain

Question: Erythromycin is contraindicated in persons with a history of severe muscle spasms. Is it yes or not?

Answer: Erythromycin is one drug of macrolide group (such as azithromycin, clarithromycin, roxithromycin, midecamycin, spiramycin, etc.).The data can be searched: erythromycin is not contraindicated for patients with a history of muscle spasms (especially skeletal muscle). However, there are reports of erythromycin increasing the contraction of smooth muscle, particularly in the gastrointestinal system. By acting through motilin receptor at the upper gastrointestinal tract that causes abdominal pain.

Drugs used to treat dogs and cats mostly similar to drugs used with people

Question: The drug which veterinarian (animal doctor) uses to treat the animal (cats, dogs) is the same type to treat people including antibiotic, antihistamine etc. Is it true or not?

Answer: Drugs are used to treat dogs and cats mostly similar to the drugs used with people. Including antibiotics and antihistamines (the dog’s wet nose is common, it is not like a cold or allergies).

But there are some medications that are the exception, such as paracetamol which will not be used as analgesic on dogs and cats because of toxicity. Especially toxic to cats, even 125 mg per dose may cause a cat to death.

Ibuprofen is very toxic to dogs, while phenylbutazone (for analgesic and inflammation) rather safes with dogs.

It is very important to consider dose of each drugs. Some medications are used in the dosage equal to people. However, some drugs are used very different doses.
Such as:- Amoxicillin; dose for common infections is 10 mg / kg by oral or injection 2-3 times per day, if the infection is severe, dose can up to 20 mg / kg 2-3 times per day.
             - Chlorpheniramine; dose for allergies and skin rashes of dog is 0.5-1 mg / kg, and cat 1-2 mg 2-3 times a day.

(1)        Extended-Release product also include in the form of controlled-release, modified-release. Often be identified by CR, MR, XL or SR after the trade name of the drug, such as Adalat CR, Diamicron MR, Xatral XL, Isoptin SR, etc.
(2)        Enteric coated product, including medications that irritate the stomach or the intestines, such as aspirin.
(3)        Sugar coated product. Most drugs of this type have bad taste.
It must be coated by sugar to cover the flavor of medicine.
(4)        Sublingual product administered sublingually.
(5)        Effervescent product. If it is broken or crushed before dissolving in water, it may affect the capability of water solubility of the drug.

The amount of calcium in calcium salts

Answer: Choosing to use calcium supplements generally depending on the type of calcium salts. The amount of calcium is contained in each type of calcium salts are not equal.

Any salt that contains much calcium will likely be absorbed into the body more than other salts. Calcium is absorbed more or less, also depending on the water solubility and disintegration of those salts.

Vitamin B-17 or laetrile

Question: What is vitamin B-17?

Answer: vitamin B-17 is another name of "laetrile", a substance found in plant especially seeds were used to make products for cancer treatment in some countries.
But it has not been tested the effectiveness of the treatment in cancer patients and has not been approved by the Food and Drug Administration of U.S.A.
Moreover, the research published in The New England Journal of Medicine in 1982 that laetrile is not effective in the treatment of cancer. And there is a danger of cyanide, which is released from that substance.

Vitamin B-17 is not essential vitamin to the human body in any way.
